29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  (a)1838 Broadwell Mill Road, Jacksonville, AL: On or about 12/19/2023, the employer exposed employees to struck-by hazards, in that forklift training was not provided to employees operating a rough terrain powered industrial truck while moving construction materials around the jobsite.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. (a) At 1838 Broadwell Mill Road, Jacksonville, AL: On or about 12/19/2023, the employer exposed employees to eye injuries in that the employer did not provide safety glasses operating nail guns.   (b) At 1838 Broadwell Mill Road, Jacksonville, AL: On or about 12/19/2023, the employer exposed employee to eye injuries in that the employer did not provide safety glasses while employees were operating circular saw to cut lumber.
