29 CFR  1910.67(c)(2)(v):  A personal fall arrest or travel restraint system that meets the requirements in subpart I of 29 CFR 1910 was not worn and attached to the boom or basket as required.  On or about December 20, 2023 the employer Colgan-Davis Inc., at jobsite located at 5644 Whiteford Rd. Sylvania OH 43560, did not ensure that employees were protected from falls while operating a Ford F600 truck (Ohio License Plate PLS 9249) equipped with an Elliott M43 brand mobile scaffold work platform (aerial lift). One employee was exposed to falls of approximately 23 feet to the ground below in that a fall protection harness and lanyard was not utilized by the employee when using the aerial lift work platform to replace parking lot light fixtures.
