You must also consider a case to meet the general recording criteria if it involves a significant injury or illness diagnosed by a physician or other licensed health care professional, even if it does not result in death, days away from work, restricted work or job transfer, medical treatment beyond first aid, or loss of consciousness. On or about December 20, 2023, at location 775 Industrial Loop Rd Ste 2, New London, Wisconsin: employer failed to record a job-related injury on the OSHA 300 log injury and illness record for employee, which occurred on or about November 6, 2023, at approximately 3:00 pm that resulted in days away from work and medical treatment beyond first aid.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about December 20, 2023, at location 775 Industrial Loop Rd Ste 2, New London, Wisconsin: employer failed to report a job-related injury within twenty-four (24) hours after the in-patient hospitalization of an employees, which occurred on or about November 6, 2023, at approximately 3:00 pm that resulted in hospitalization, days away from work, and medical treatment beyond first aid.
