29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers was equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements:  (a) Hospital Area - On or about December 20, 2023, employees accessing the mezzanine above the hospital area were exposed to a fall hazard in that the enclosed stairs did not have a handrail on each side.  (b) Furniture Preparation Area - On or about December 20, 2023, employees accessing the mezzanine above the furniture preparation area were exposed to a fall hazard in that the stairs were open on one side and did not have a stair rail system with a handrail on the open side and one handrail on the enclosed side.
