29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: On or about December 20, 2023, in Malvern at a worksite located at 825 Baker Street, employees were exposed to struck by hazards in the following instances: a) Employees working from the elevated bucket of an Altec bucket truck were cutting limbs from a tree and were not wearing hard hats; b) Employees removing limbs and feeding the chipper were not wearing hard hats.
