All deficiencies such as listed were not carefully examined and determination was not made as to whether they constitute a safety hazard:  All functional operating mechanisms for excessive wear of components.  On or about December 20, 2023, employees engaged in cement truck shroud repairs at the Consolidated Concrete Co. facility located at 2000 N Baltimore Ave, Hastings, NE, were exposed to crushing and struck by hazards because the hoist chain they were using to support the shroud had not been inspected and removed from service due to the following defects:  a.  A bolt of unknown material strength is being used in place of a clevis pin that is supporting the load.    b.  The bolt's threads were not engaged fully, weakening the structural integrity of the clevis.  c.  Multiple hoist hooks are missing safety latches.
