At a temporary worksite located at, or near, 29 Pine in the Wood in Port Orange, Florida 32129: On or about December 19, 2023, the employer exposed employees to struck-by and crushed-by hazards, in that the wood blocks used to support a truck-mounted crane's outriggers were not of sufficient width and length to prevent shifting or toppling under load, as employees utilized the crane during tree-felling operations.
