29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A) through (c)(4)(ii)(D). a. On or about December 12, 2023, the employer developed energy control procedure for the Line 11 Siemens 410 belt-on-roller conveyor system, did not clearly and specifically include a specific statement of the intended use of the procedure [(c)(4)(ii)(A)], specific procedural steps for shutting down the equipment, identifying the specific disconnect switch to isolate the electrical energy, steps for blocking and securing the tension and/or potential energy remaining in the system, specific steps the conveyor to control the hazardous energy [(c)(4)(ii)(B)], specific steps for the removal and/or transfer of lockout devices [(c)(4)(ii)(C)], and specific steps to verify the effectiveness of the lockout measures [(c)(4)(ii)(D)].

29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed in accordance with 29 CFR 1910.147(f)(1)(i) through (f)(1)(v): a. On or about December 12, 2023, and at times prior, the employer did not develop procedures for employees performing equipment testing and positioning activities to ensure effective employee protection from energized and moving equipment. On December 12, 2023, an employee was caught in a running conveyor while performing maintenance troubleshooting at the underside of the Siemens 410 belt-on-roller conveyor located at Line 11 in the packing area of the DC1 warehouse.
