29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered in operational.  At the workplace located at 23420 W 79th street, Shawnee, Kansas, employees engaged in maintenance activities on multiple custom injection molding machines were exposed to the release of uncontrolled hazardous energy. The employer had not established an energy control program consisting of an energy control procedure, employee training and periodic inspections that ensured both affected and authorized employees understood the different elements of the control of hazardous energy required by the standard.

29 CFR  1910.1200(e)(1):Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   At the workplace located at 23420 W 79th street, Shawnee, Kansas, the employer failed to develop a written hazard communication program to describe how the program would be implemented to include proper training, labeling, and the administration of safety data sheets, to address their chemicals such as  Slide knock out mold release, quick mold cleaner which are flammable, carbon black, which is an irritant.
