29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Zerand 2 Printer Die Cutter Machine ("Z2") Area - On and prior to December 15, 2023, employees did not utilize lockout/tagout procedures when performing the task of cleaning the print mat roller on Zerand 2 Printer Die Cutter Machine ("Z2") (Mfg: KYMC, Model: 612S, Serial #4793) exposing the employees to caught-in and pinch-point hazards.

29 CFR 1910.147(c)(7)(iii)(B): Additional retraining was not provided whenever a periodic inspection under paragraph (c)(6) of this section reveals, or whenever the employer has reason to believe that there are deviations from or inadequacies in the employee's knowledge or use of the energy control procedures: a) Zerand 2 Printer Die Cutter Machine ("Z2") Area - On and prior to December 15, 2023, employees did not utilize lockout/tagout procedures when performing the task of cleaning the print mat roller on Zerand 2 Printer Die Cutter Machine ("Z2") (Mfg: KYMC, Model: 612S, Serial #4793) exposing the employees to caught-in and pinch-point hazards. Employees were trained on lockout/tagout procedures, but were allowed to perform the cleaning the print mat task without utilizing the lockout/tagout procedures. Retraining was not provided to these employees when they deviated from the company's energy control procedures for this task.
