29 CFR 1926.453(b)(2)(v):A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: At a commercial worksite located at 16035 NW Hwy 45, Parkville MO -- An employee in an aerial lift was not tied off with a lanyard to the basket. This exposed the employee to being catapulted from the aerial lift to ground level. This was documented on or about 12/20/2023.
