Location: 208 Valley Road New Canaan, Connecticut   a) On or about December 21, 2023, employees were exposed to cave-ins  while working in an over 12 foot deep trench with vertical walls and no cave-in protection.  b) On or about December 22, 2023, employees were exposed to cave-ins while working in an over 12 foot deep trench with vertical walls and no cave-in protection, which resulted in the fatality of the foreman.

Location: 208 Valley Road New Canaan, Connecticut   a) On or about December 21, 2023, employees were exposed to struck-by and engulfment hazards while working around and in a trench with a Kubota KX080-4 excavator that was positioned on the edge of the trench with no restraining devices to prevent it from falling or rolling into the trench.   b) On or about December 22, 2023, employees were exposed to struck-by and engulfment hazards while working around and in a trench with a Kubota KX080-4 excavator that was positioned on the edge of the trench with no restraining devices to prevent it from falling or rolling into the trench.
