29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:  a) Residential construction: On 12/22/2023, the employer exposed employees to electric shock and burn hazards, in that employees engaged in roofing activities came in close proximity with three insulated service cables on the roof as these wires were not de-energized or covered prior to the start of work.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  a) Residential construction: On 12/22/2023, the employer exposed employees to fall hazard of 17-feet, 2-inches to the lower level, in that employees were not provided with a fall protection system while roof underlayment and repairing rotted wood without a fall protection system on a 5/12 pitched roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Residential construction: On 12/22/2023, the employer exposed employees to fall hazards of approximately 17-feet, 2-inches to the lower ground level, in that the extension ladder was not extended at least 3-feet  past the eave to provide a safe means of access/egress on and off the roof.

29 CFR  1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:  a) Residential construction: On 12/22/2023, the employer exposed employees to fall hazards, in that a competent person did not train employees on how to erect, maintain, disassemble, and inspect the fall protection systems to be used.
