Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to flash fire and explosion hazards:  (a) On December 22, 2023, at or near 63rd Street NW and 131st Avenue NW at the Jennifer 26-35 #2TFH wellsite near Epping, North Dakota, an employee was exposed to fire and explosion hazards while conducting well servicing operations where there was a release of flammable vapors from the well which were ignited by the presence of ignition sources    Note: Among other methods, feasible and acceptable abatement methods to correct this hazard include but are not limited to:  1. Follow the American Petroleum Institute Recommended Practice 54, Occupational Safety and Health for Oil and Gas Well Drilling and Servicing Operations section 5.2 "Fire Safety". 2. Follow the American Petroleum Institute Recommended Practice 99, Flash Fire Risk Assessment for the Upstream Oil and Gas Industry section 4 - Where potential flash fire hazards exist, employers shall conduct a risk assessment and utilize controls to mitigate the risk of flash fire injury.
