(a) Jobsite at 244 Frontage Road in Minerals Wells, WV: On or about December 28, 2023 and times prior, the safety nets being used as fall protection had panels that had openings larger than 6 inches, exposing employees to falls of up to 25 feet to the concrete ground.

29 CFR 1926.502(c)(5): Defective nets shall not be used. Safety nets shall be inspected at least once a week for wear, damage, and other deterioration. Defective components shall be removed from service. Safety nets shall also be inspected after any occurrence which could affect the integrity of the safety net system. (a) Jobsite at 244 Frontage Road in Minerals Wells, WV: On or about December 28, 2023 and times prior, the safety nets being used as fall protection were defective, in that, there were multiple sections of the nylon rope or mesh broken and frayed, exposing employees to fall of up to 25 feet to the concrete ground.

The area below where the roofing work was occurring was not barricaded and warning signs were not posted.
