29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. Location: 23 Putnam Hill Road Lyndeborough, NH On or about December 7, 2023, an employee partially amputated their left ring finger while operating a Reichdrill well drilling rig. The employer was notified of the amputation on December 7, 2023, and did not report the amputation to OSHA within 24 hours. OSHA was notified of the amputation on December 21, 2023.
