OSH ACT of 1970 Section (5)(a)(1)   The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees  a) 220 Old West Penn Ave, Robesonia, PA - Building 1/Line 1 - Combustible materials accumulated in an interior rain gutter, and the employer had not conducted housekeeping to remove the deposits of combustible materials from the interior rain gutter prior to permitting hot work activities.  Employees of a sub-contactor were torch cutting bolts, and subsequently started a fire in the rain gutter, on or about December 27, 2023.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.252(a)(2)(iv):  Before cutting or welding is permitted, the area was not inspected by the individual responsible for authorizing cutting and welding operations, and he did not designate precautions to be followed in granting authorization to proceed:   a) 220 Old West Penn Ave, Robesonia, PA - Building 1/Line 1 - Employees of a sub-contactor were torch cutting bolts, and subsequently started a fire in an interior rain gutter.  The management from the host employer who authorized the hot work did not inspect an interior rain gutter that accumulated with combustible materials from prior work operations that included, but were not limited to, a pre-hot work wash down of the structure's interior walls, on or about December 27, 2023.   b) 220 Old West Penn Ave, Robesonia, PA - Building 1/Line 1 - Employees of a sub-contactor were torch cutting bolts, and subsequently started a fire in an interior rain gutter.  The management from the host employer who authorized the hot work did not take measures to clean the interior rain gutter of the combustible contents that accumulated from prior work operations that included, but were not limited to, a pre-hot work wash down of the structure's interior walls, on or about December 27, 2023.
