29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Automotive building; On or about December 27, 2024, and dates prior, specific procedures were not developed and documented to identify the applicable energy, ex. electrical and thermal, and the steps to be used to control the energy, before employees performed maintenance activities on any of the injection molding machines. At the time of this inspection, molding press B-5 was locked out by a maintenance employee. (b) Automotive building; On or about December 27, 2024, and dates prior, specific procedures were not developed, documented, and utilized for machines such as but not limited to molding presses C-4 and F-4 before employees performed servicing activities. At the time of this inspection, molding presses C-4 and F-4 were not locked out while mold technicians performed servicing activities of changing tooling on the injection molding machine.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a) Automotive building, shipping/receiving area; On or about December 27, 2023, and dates prior, a Yale brand forklift (model: ESCO30FAN36TE082) was not equipped with markings for each attachment used on it, such as a Vestil Manufacturing Corporation forklift boom (model: LMS-EBT-46-4).
