29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. U.S Postal Service, Los Angeles P&DC: On and/or about December 21, 2023 employee(s) were processing an incoming delivery at dock 45. An employee was hospitalized due to a fall which occurred over 4 feet in height. A late notification to OSHA was made on December 27, 2023. The U.S Postal Service was previously cited for a violation of this occupational safety health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1694817, citation number 1, item number 1, and was affirmed as a final order on January 16, 2024, with respect to a workplace located at 300 N. 10th St. Killeen, TX 76540.

29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. Dock 40-53: On December 21, 2023 employee(s) processed incoming outgoing mail deliveries. The employer did not provide training regarding fall hazards for the dockboards/ dock leveler where fall(s) from 4 feet or greater could occur and in accordance with this subpart for each employee who worked at the docks. This condition potentially exposed employee(s) to fall hazard(s).
