On or about December 11, 2023, at the jobsite located at 755 Glenhurst Ave, Willowick OH, 44095; during waterproofing operations in a trench at a residential home, the employer did not ensure portable ladders were inspected prior to use by a competent person, in that the ladder in use had damaged rungs.
