29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system:   On or about December 28, 2023, and at times prior thereto, employees performing roofing repairs more than 6-feet above the lower level did so without the use of fall protection, exposing them to fall hazards.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident.    The employer did not report a fatality that occurred on or about December 28, 2023, at jobsite located at 775 North Ash Street, Muenster, Texas 76252 within the required 8-hours after knowing of the fatality.  The incident was reported to OSHA on January 1, 2024.
