29 CFR 1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Seneca Millwork Inc. on or about January 3, 2024, at the address of 300 Court Place Fostoria, OH: The employer did not ensure powered industrial truck operators performance was evaluated and documented at least every three years. Due to this condition, employees operating forklifts in the production area were not current on their performance evaluation, exposing employees working on or around the forklifts to potential struck by hazards.
