At buildings 100, 200, 300, 400, and 1400 on 5/12 pitched second story residential roofs at the silas Oaks Apartments located at 1120 Silas Drive Southwest in Live Oak Florida, 32064: On January 3, 2024, the employer exposed employees to fall hazards ranging from 6 to 24-foot, in that fall protection was not utilized by employees, while performing roofing work .
