29 CFR  1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas.    On or about December 15, 2023, and at times prior thereto, employees conducted residential construction and the construction site was not kept clear of scrap lumber, protruding nails and other rubbish.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502  On or about December 15, 2023, and at times prior thereto, employees conducted roof decking and installation operations, at heights of greater than 6 feet above a lower level, and a fall protection system was not in place to protect from fall hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about December 15, 2023, and at times prior thereto, employees used a ladder to access the roof that did not extend at least 3 feet above the roof landing.

29 CFR  1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed.    On or about December 15, 2023, and at times prior thereto, employees conducted exterior siding installation operations from closed self-supporting stepladders leaning against an exterior wall and the ladders were not used for its intended purposed design.
