Hearthside Food Solution, LLC on or about February 2, 2024 at the location of 312 Rader Rd, Mc Comb, OH 45858: The employer did not ensure all exit routs were unobstructed. Due to this condition, employees tasked with working on the Graham Cracker Line (Line 9) Wrapper A and B were required to pass under permanent conveyers approximately 4' from ground level to access emergency exits. Employees working in this condition were exposed to deficient emergency evacuation access subjecting them to potential injury.

Employees performing in-operational cleaning of the machine in proximity of the un guarded nip point were exposed to caught-in hazards. b).
