29 CFR  1910.179(b)(5):  The rated load of the crane(s) was not plainly marked on each side of the crane, and crane(s) with more than one hoisting unit did not have the rated load marked on each hoisting unit or load block.    On or about January 3, 2024, and at times prior to, inside of the facility on the southwest side, an overhead crane with 2 hoists, had no markings on each side of the crane showing the crane's rated load and the rated load of 1 hoist of the overhead crane was not clearly marked.  The overhead crane was used by employees to lift heavy equipment and parts.

On or about January 3, 2024, and at times prior to, inside of the facility on the southwest side, overhead crane pendant controls had damage which exposed electrical wires.  Employees used the pendant controls to operate an overhead crane to lift heavy equipment and parts.

Employees used the cylinders for torch cutting and welding operations.

On or about January 3, 2024, and at times prior to, outside at the south wall of the facility, at a storage cage, valve protection caps were not installed on 2 stored cylinders of Compressed Oxygen (UN1072) and 1 stored cylinder of Compressed Gas N.O.S. (UN1956).  Employees used compressed gas cylinders for torch cutting and welding operations.

On or about January 3, 2024, and at times prior to, outside at the south wall of the facility, in a single storage cage, compressed oxygen cylinders were stored next to acetylene cylinders.  Employees used compressed oxygen cylinders and acetylene cylinders for torch cutting operations.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  On or about January 3, 2024, and at times prior to, outside at the south wall of the facility, compressed oxygen cylinders were stored in a cage that was less than 20 feet from a storage cage of liquified petroleum gas (LPG) cylinders.  Employees used compressed oxygen cylinders for torch cutting operations.
