Employees were framing the residential building approximately 11.5 feet to the ground level without the use of a fall protection system.  b) Jobsite located at 1406 East Moyamensing Avenue in Philadelphia, Pennsylvania- On or about January 3, 2024, the employer did not ensure an employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net system, or personal fall arrest system.  The employer allowed an open hole to in the floor to be left uncovered, thereby exposing employees to fall hazards of approximately 9 feet to the basement below.
