(a) Worksite: On or about 01-04-2024, an employee was exposed to respirable silica dust while performing supervisory oversight without the use of engineering controls, work practices, and respiratory protection specified for the task on Table 1 section (iv), in that the saw was utilized without an integrated water delivery system.
