29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about January 4, 2024, at 475 SW 27th Ave., Fort Lauderdale, FL 33312, employees accessed formwork structures for removal of shoring in cantilever slabs, and the employer did not ensure that workers were provided a means of fall protection from a 12 ft. gall hazard elevation to ground level.  Or in the alternative:  29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. On or about January 4, 2024, at 475 SW 27th Ave., Fort Lauderdale, FL 33312, employees accessed formwork structures for removal of shoring in cantilever slabs, and the employer did not ensure that workers were provided a means of fall protection from a 12 ft fall hazard elevation to ground level.

29 CFR 1926.703(a)(1): Formwork was not designed, fabricated, erected, supported, braced and maintained so that it would be capable of supporting without failure all vertical and lateral loads that could reasonably be anticipated to be applied to the formwork.  a. On or about January 4, 2024, at  475  SW 27th Ave., Fort Lauderdale, FL 33312, employees were exposed to fall, struck-by, and caught-in hazards  during the removal of shoring material under cantilever slabs, in that the employer did not ensure the stability of the form and shoring structures through adequate design, support, bracing and maintenance to prevent the collapse of the formwork structure and the entrapment of the workers.  Or in the alternative:  29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.
