29 CFR  1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.   a)  480 Bank Lane, Dover, DE 19904, on 1/4/2024, employees were permitted to work on fabricated metal scaffold platforms 4-feet below occupied scaffold platforms and were not protected from falling objects, including but not limited to (material/tools that could fall on employees).

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) 480 bank Lane, Dover, DE 19901: On or about 1/4/2024, employees observed working on the roof with a low-pitched roof, approximately 15 feet above lower level, without any fall protection. CSHO observed the same employees crawling down the roof to the ladder without any fall protection.
