29 CFR 1910.28(b)(5)(i): The employer did not ensure that each employee on a runway or similar walkway is protected from falling 4 feet (1.2 m) or more to a lower level by a guardrail system. On or about January 4, 2024, employees installing handrails on an upper level of a walkway of the plant were not protected by guardrail systems, safety net systems, or personal fall protection systems, exposing the employees to a fall hazard of over 4-feet.
