On or about 01/04/2024, at 4681 107th Circle North, Clearwater, Florida, 33762, an employee was operating CNC (Computer Numerical Control) Lathe number 3, to cut/fabricate various metal components, during which time the access door safety interlock switch/primary safety guard was bypassed/inoperable; therefore exposing the employee/lathe operator to amputation hazards.       c. On or about 01/04/2024, at 4681 107th Circle North, Clearwater, Florida, 33762, an employee was operating CNC (Computer Numerical Control) Lathe number 5, to cut/fabricate various metal components, during which time the access door safety interlock switch/primary safety guard was bypassed/inoperable; therefore exposing the employee/lathe operator to amputation hazards.       d. On or about 01/04/2024, at 4681 107th Circle North, Clearwater, Florida, 33762, an employee was operating CNC (Computer Numerical Control) Lathe number 6, to cut/fabricate various metal components, during which time the access door safety interlock switch/primary safety guard was bypassed/inoperable; therefore exposing the employee/lathe operator to amputation hazards.       e.
