29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about 01/04/2024, at 4681 107th Circle North, Clearwater, Florida, 33762, an employee was operating CNC (Computer Numerical Control) Lathe number 2, to cut/fabricate various metal components, during which time the access door safety interlock switch/primary safety guard was bypassed/inoperable; therefore exposing the employee/lathe operator to amputation hazards. b. On or about 01/04/2024, at 4681 107th Circle North, Clearwater, Florida, 33762, an employee was operating CNC (Computer Numerical Control) Lathe number 3, to cut/fabricate various metal components, during which time the access door safety interlock switch/primary safety guard was bypassed/inoperable; therefore exposing the employee/lathe operator to amputation hazards. c. On or about 01/04/2024, at 4681 107th Circle North, Clearwater, Florida, 33762, an employee was operating CNC (Computer Numerical Control) Lathe number 5, to cut/fabricate various metal components, during which time the access door safety interlock switch/primary safety guard was bypassed/inoperable; therefore exposing the employee/lathe operator to amputation hazards. d. On or about 01/04/2024, at 4681 107th Circle North, Clearwater, Florida, 33762, an employee was operating CNC (Computer Numerical Control) Lathe number 6, to cut/fabricate various metal components, during which time the access door safety interlock switch/primary safety guard was bypassed/inoperable; therefore exposing the employee/lathe operator to amputation hazards. e. On or about 01/04/2024, at 4681 107th Circle North, Clearwater, Florida, 33762, an employee was operating CNC (Computer Numerical Control) Lathe number 7, to cut/fabricate various metal components, during which time the access door safety interlock switch/primary safety guard was bypassed/inoperable; therefore exposing the employee/lathe operator to amputation hazards. f.
