29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Employee(s) were exposed to a fall hazards. This was observed on or about January 3, 2024, at or about 361 South St., Grafton, Nebraska. Employee(s) were using the Werner portable ladder to access and egress the roof. This ladder did not extend at least 3 feet above the roof as required by this sub-part.
