29 CFR  1926.452(g)(1): Each bracket, except those for wooden bracket-form scaffolds, was not attached to the supporting formwork or structure by means of one or more of the following: nails; a metal stud attachment device; welding; hooking over a secured structural supporting member, with the form wales either bolted to the form or secured by snap ties or tie bolts extending through the form and securely anchored; or, for carpenters' bracket scaffolds only, by a bolt extending through to the opposite side of the structure's wall.   115 Ocean Drive West, Stamford CT: On or about January 5, 2024, the carpenter was working on a carpenter's bracket scaffold where each bracket was only attached to the structure by a single nail instead of a bolt extending through the entire wall, exposing the worker to a scaffold collapse at over 19 feet above the ground.

29 CFR  1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.   115 Ocean Drive West, Stamford CT: On or about January 5, 2024, the carpenter was working on a 24 foot long scaffold platform that was installed so that it extended at least 3 feet beyond its support exposing the worker to a tipping and falling hazard.
