29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about January 4, 2024, the side rails of a portable ladder used by employees to access the roof did not extend at least three (3) feet above the landing surface. This exposed employees to a fall hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about January 4, 2024, employees were performing decking work activities on the roof of a commercial building and were not in use of fall protection. This exposed the employees to fall hazards greater than six (6) feet to the ground below. Arzate Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1622463, citation number 1, item number 1 and was affirmed as a final order on March 23, 2023, with respect to a workplace located at 12817 SW 26th St, Yukon, OK.
