29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  a. On or about January 4, 2024, at the above addressed jobsite, an employee conducting carpentry operations on a wooden man-made box was elevated by a JLG Skytrak 8042 forklift, that was not secured to forks, thereby exposing the employee to approximately 10 feet fall hazard.

On or about January 4, 2024, at the above addressed jobsite, employees performing carpentry operations from an elevated power industrial vehicle/Skytrak 8042 forklift, approximately 10 feet above the ground, was not protected by guardrail systems along all open sides of platform, thereby exposing the employee to fall hazards.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval:  a. On or about January 4, 2024, at the above addressed jobsite, the employer was using an unapproved attachment (wooden job made box) on the forks of a power industrial vehicle/JLG Skytrak 8042 forklift to hoist an employee to the roof of a new residential construction. This practice could affect the capacity of the equipment and the manufacturer's written approval was not obtained, thereby exposing the employee to a struck by hazards due to forklift tip over.
