29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about January 5, 2024, at the #13 Deep Well, employees were performing servicing and maintenance on equipment including, but not limited to, a vertical turbine pump. The lockout and tagout procedure did not include specific procedural steps to control all energy sources, including mechanical.
