At a residential worksite, located at 2396 Hilly Road, Jacksonville, FL 32209: On or about January 9, 2024, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees while using pneumatic nail guns to install roofing materials.

On the roof of a residential worksite, located at 2396 Hilly Road, Jacksonville, FL 32209: On or about January 9, 2024, the employer exposed employees to 18-foot fall hazards, in that, fall protection was not utilized by employees while performing roofing work.

At the Porch Area of a residential worksite, located at 2396 Hilly Road, Jacksonville, FL 32209: On or about January 9, 2024, the employer exposed employees to fall hazards ranging from 6 to 9-foot, in that a portable extension-type ladder had structural defects, as it was utilized by employees to access the roof.
