29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again or the employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired: Exterior Dock Stairs: On, or about, January 9, 2024, the wrought iron stair railing provided on the left side descending had been broken off after the stairs were hit by a trailer. The stairs were used daily.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Sorting Area: On, or about January 9, 2024, a relocatable power tap was used to energize five electronic components such as a CPU, monitor and scanner. All of the components were provided with an equipment grounding conductor. The relocatable power tap was energized by a two-wire extension cord that did not include an equipment grounding conductor.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: Shop Area: On, or about, January 9, 2024, protective railings were installed in the working space of Panel RP-1, a General Electric 480/277 volt panel board. The working space required in Table S-1 was 42 inches, the railing was measured to be 37 inches from the face of the panel enclosure.
