29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 9, 2024, and at times prior thereto, the employer did not prevent employees from entering and in close proximity to rotating parts of equipment. Employees were exposed caught-in and amputation hazards. 1. In the Roll Form Cell 12 Department the Dahlstrom Machine Safety interlocks were broken and ineffective. 2. In Cell 1, the Saw guard with a hole for cleaning was ineffective.
