29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired:   On or about January 10, 2024 employees engaged in warehousing activities in Warehouse #9 of Hansen's Truck Salvage, Inc. located at 5702 South 60th Street, Omaha, Nebraska were exposed to trip and fall hazards in that the employer did not prevent employees from accessing damaged walking and working surface areas. Specifically, Warehouse #9 expanded metal flooring had pieces of unsecured plywood sheeting partially covering the original flooring creating an uneven walking surface of slip and trip hazards to employees accessing this area.

29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury:  On or about January 30, 2024, employees engaged in warehousing activities in Warehouse #4 at Hansen's Truck Salvage, Inc. located at 5702 South 60th Street, Omaha, Nebraska were exposed to a fall hazard in that the employer did ensure mobile ladders were inspected for defects at the beginning of the work shift prior to use.  Instances included but were not limited to:  a. Mobile ladders #1 and #2 with structural damage, such as bent framing and b. Mobile ladder #2 with one bent wheel that prevented wheel(s) from locking.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:   On or about January 10, 2024, employees performing vehicle dismantling in the Dismantling Department of Hansen's Truck Salvage, Inc. located at 5702 South 60th Street, Omaha, Nebraska were exposed to fire and electrical shock hazards related to overloaded 120 volt electrical in that the employer has employees using the Milwaukee Reciprocating Sawzall that is plugged into a green flexible cord which is in turn plugged into a portable power tap designed as means of distributing power to computer peripherals.

Instances included but were not limited to:  a. Milwaukee Sawzall reciprocating saw with cuts and damage to the exterior power cord insulation, and b. Green flexible extension cord with damage to the exterior insulation near the plug at which the Milwaukee Sawzall reciprocating saw is plugged in
