29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. On or about January 10, 2023: employees operated powered industrial trucks with damage including, but not limited to, chunks of tire missing, severe splitting of tire sidewalls and tires pulling away from the rims. a) Plant 1 located at 2430 East Glendale Ave, Appleton, Wisconsin: F-048 Toyota Forklift (Model: 7FBCU25 Serial: 63919) was operated with damaged tires. Damage to the tires was found to include, but was not limited to, severe splitting of sidewalls and tires pulling away from the rims, which could result in the tires to lose traction and possibly expose forklift operators to tip-over hazards. b) Plant 2 located at 2441 East Glendale Ave, Appleton, Wisconsin: C-001 CAT Roll Clamp Forklift (Model: GC55K Serial: AT88B10757) was operated with damaged tires. Damage to the tires was found to include, but was not limited to, severe splitting of sidewalls and tires pulling away from the rims, which could result in the tires to lose traction and possibly expose forklift operators to tip-over hazards. c) Plant 3 located at 2239 East Glendale Ave, Appleton, Wisconsin: F-007 Linde (Model: 725CL Serial: A11347X00016) was operated with damaged tires. Damage to the tires was found to include, but was not limited to, chunks of tire missing, severe splitting of sidewalls and tires pulling away from the rims, which could result in the tires to lose traction and possibly expose forklift operators to tip-over hazards. d) Plant 3 located at 2239 East Glendale Ave, Appleton, Wisconsin: F-006 Linde Forklift (Model: E25CL Serial: A11347X00014/2020) was operated with damaged tires. Damage to the tires was found to include, but was not limited to, chunks of tire missing, severe splitting of sidewalls and tires pulling away from the rims, which could result in the tires to lose traction and possibly expose forklift operators to tip-over hazards. e) Plant 3 located at 2239 East Glendale Ave, Appleton, Wisconsin: F-005 Linde Forklift (Model: E25CL Serial: A11347X00015/2020) was operated with damaged tires. Damage to the tires was found to include, but was not limited to, chunks of tire missing, severe splitting of sidewalls and tires pulling away from the rims, which could result in the tires to lose traction and possibly expose forklift operators to tip-over hazards.
