29 CFR 1910.28(b)(8): The employer did not provide fall protection systems for repair pits, service pits, and assembly pits:  On or about January 10, 2024 , Employees engaged in waste disposal activities at the waste disposal company located at 1839 East 4th Street, Grand Island, Nebraska, were exposed to an approximately 5 foot fall hazard in that the service pit in the Shop Area did not have a fall protection system in place to protect open sides.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":  On or about January 10, 2024, Employees engaged in waste disposal activities located at 1839 East 4th Street, Grand Island, Nebraska, were exposed to a fire hazard in that the door in the Shop Area that was intended to be used as an exit was not marked by a sign reading "Exit".

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:  The employer failed to maintain injury and illness logs. This violation was most recently documented at a worksite located at or near 1839 East 4th Street, Grand Island, Nebraska  on or about January 10, 2024. The employer did not ensure that the OSHA Form 300 for the calendar year of 2023 was completed as required, in that;   Block F of the OSHA 300 form was not completed with sufficient detail.  Instances of this failure include but are not limited to:  i.	Case No. 1, January 4th, listing only "Finger Strain" in Block F. ii.    Case No. 2, September 22nd, listing only "Lower Back Strain" in Block F.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):                                                         Employees engaged in activities at the commercial site located at 1839 East 4th Street, Grand Island, Nebraska, were exposed to amputation and electrical hazards in that the employees were not provided with an energy control program by the employer that met the requirements of 1910.147, including but not limited to;   i)	Authorized employees servicing equipment, ii)	Non-authorized employees whose work operations may be in a area where energy control procedures are being utilized and; iii)	Specific energy control procedure for the cardboard baler.
