29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer shall obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels. On or about and at times prior to February 6, 2024, at the establishment located at 2441 E Glendale Ave, Appleton, Wisconsin; in Plant 3, the employer did not ensure that a new audiogram was obtained annually for each employee with noise exposure that exceeded 83.4 decibels measured on an A-weighted scale (dBA) in the following instances: a) On or about February 6, 2024, an Operator employee operating the Webco was exposed to a 10-hour time-weighted average (TWA) sound level of 89.8 dBA, or equivalently, a noise dose of 98.2%. This exceeds an 10-hour TWA of 83.4 dBA, or equivalently, a noise dose of 50%. The sampling was performed for 564 minutes. b) On or about February 6, 2024, an Operator employee operating the Jagenberg 1 was exposed to a 10-hour TWA sound level of 90.6 dBA, or equivalently, a noise dose of 109.6%. This exceeds an 10-hour TWA of 83.4 dBA, or equivalently, a noise dose of 50%. The sampling was performed for 558 minutes. c) On or about February 6, 2024, a Rewind Operator employee operating the Jagenberg 2 was exposed to a 10-hour TWA sound level of 92 dBA, or equivalently, a noise dose of 132.4%. This exceeds an 10-hour TWA of 83.4 dBA, or equivalently, a noise dose of 50%. The sampling was performed for 563 minutes.
