29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) Energy Center - On or about January 10, 2024, the employer did not provide a suitable eyewash for quick drenching or flushing of the eyes and body outside the machinery room door for employees who work with ammonia (corrosive). The failure to provide functioning eyewash stations can cause chemical burns to the eyes or skin while working in an area where anhydrous ammonia could be inadvertently released. Abatement Certification Required.

29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: Engine Room - On or about January 10, 2024, the employer did not implement safe work practices when opening process piping. The ammonia service line was not confirmed to be in vacuum per SOP-7 - LINE BREAKING OF AMMONIA PIPING & SYSTEM prior to opening the piping exposing the employees to ammonia vapors, fire, and explosion hazards. Abatement Certification Required.
