Acura of Berlin, 224 Berlin Turnpike, Berlin, CT: On or about January 11, 2024, an employee who exited and was struck by the moving fork truck he was operating, had not been provided with a practical evaluation of his driving abilities during refresher training.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): Acura of Berlin, 224 Berlin Turnpike, Berlin, CT: On or about January 11, 2024, and at times prior thereto, a Clark forklift, model # C30MB, serial # C30MB-230-0800, was found to have a missing catch plate on the outer casing in order to use the metal clamp to properly secure the propane tank, exposing employees to struck by hazard.
