On or around January 11, 2024, an employee was exposed to a to struck-by hazard while operating a pneumatic nail gun without eye protection.

29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables were used.   On or about January 11, 2024, an employee was exposed to an electric shock hazard while using a damaged flexible cord at the worksite:  Instance 1: While the EE was using a yellow in color extension cord with exposed copper wiring at both ends of the cord.  Instance 2: While the EE was using the Makita circular saw with a damaged and spliced power cord.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about January 11, 2024, an employee was exposed to a fall hazard of more than 6 feet while working on the roof without a fall protection system.    Adrian Castrol Solano was previously cited for a violation of the Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1502155, citation number 1, item number 1 and was affirmed as a final order on 4/30/2021 with respect to a workplace located at 1848 Stoney Sunset, Dickinson, TX 77539.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about January 11, 2024, an employee was exposed to a fall hazard when accessing the southeastern roof with a ladder whose side rails did not extend at least three feet above the landing surface.   Adrian Castrol Solano was previously cited for a violation of the Occupational Safety and Health Standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1502155, citation number 2, item number 1 and was affirmed as a final order on 4/30/2021 with respect to a workplace located at 1848 Stoney Sunset, Dickinson, TX 77539.
