On or about January 10, 2024, the employer had not correctly and completely entered workplace injuries or illnesses on the OSHA Form 300 for calendar year 2023.  (a) On or about November 20, 2023, an employee was struck in the head by a hold down bar which resulted in days away from work.  The employer did not enter the case on the form.
