29 CFR 1910.28(b)(3)(iv): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toe boards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  a) On or about 01/10/2024 in the Foam Fabrication Area; employees were exposed to a fall hazard of approximately 7 feet when the top of the ladderway used to access the mezzanine was open without any protection from falling into the opening at the top of the ladder. 29 CFR  1910.29(b)(1): The employer did not ensure that guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members, that are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section.  a) On or about 01/10/2024, in the Foam Fabrication Area, the mezzanine's guardrail system had a missing top rail exposing employees to falling from a height of at least 7 feet when going on the mezzanine to retrieve or stock materials.  b) On or about 01/10/2024, in the Production Area, the Hooper-Swift's guardrail system on the elevated platform had a missing top rail exposing employees to falling from a height of at least 4 feet when going on the platform to service the machine.   ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) On or about 01/10/2024 throughout establishment; where employees perform servicing and/or maintenance activities such as, but not limited to: setting up the S+S Slitter and Hooper-swift machine and changing the blade on the Foam splitter saws. The employer has not developed machine-specific energy control procedures, nor has the employer trained or periodically reviewed employees performing covered operations where the employees were exposed to potentially hazardous energy sources. 29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a) On or about 01/10/2024 throughout establishment; where employees perform servicing and/or maintenance activities such as, but not limited to: setting up the S+S Slitter and Hooper-swift machine and changing the blade on the Foam splitter saws. Employees were not trained to recognize hazardous energy sources in the workplace and the means and methods (procedures) used to control those hazardous energy sources during servicing and/or maintenance of the machines and equipment in the workplace.
