29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) In the facility: On or about 01/11/2024, and at times prior to, the employer exposed employees to struck-by hazards, in that employees operated the Hamech g25st-16 forklift where holes were made to a fork and a trailer hitch ball was attached to maneuver boat trailers without the manufacturer's prior written approval. b) In the facility: On or about 01/11/2024, and at times prior to, the employer exposed employees to struck-by hazards, in that employees operated the Toyota model 7fgcu25 forklift where holes were made to a fork and a trailer hitch ball was attached to maneuver boat trailers without the manufacturer's prior written approval.
